As a Social Services Assistant at Oak Grove Center in Waterville, ME, you will play a vital role in enhancing the lives of our patients/residents. This part-time position offers a unique opportunity to make a meaningful difference in the lives of others.

Key Responsibilities:
  • Collaborate with our interdisciplinary team to promote and protect patient/resident rights and well-being.
  • Develop and implement individualized plans of care to address social service needs.
  • Provide support and information to patients/residents, families, and significant others to improve quality of life.
  • Facilitate patient/resident discharge planning activities, including education, documentation, and referrals.
  • Establish and maintain relationships with community-based agencies and services to benefit our patient/resident population.

Requirements:
