Caregiver for Seniors

The Los Angeles Jewish Home is seeking a compassionate and skilled Caregiver for Seniors to join our team at the Brandman Centers for Senior Care (BCSC). As a Caregiver, you will provide high-quality, personalized care to our participants, ensuring their dignity, comfort, and well-being.
Key Responsibilities:
- Assist participants with bathing, grooming, toileting, and feeding in a safe and respectful manner.
- Accompany participants to medical appointments and provide transportation as needed.
- Assist with translation services to other departments as required.
- Monitor vital signs, such as weight, blood pressure, and pulse, as ordered by the clinic.
- Maintain personal care supplies, including incontinence supplies, linen, and personal grooming items.
- Provide laundry service in the PACE Center as agreed upon by the Interdisciplinary Team (IDT).
- Lead transitional exercise programs in coordination with the rehabilitation department.
- Assist with leading activities and the walking group in coordination with the activities department.
- Monitor participants who need supervision while smoking at the center.
- Maintain a clean and sanitary environment in the PACE Center at all times.
- Provide assistance with meals as identified by the IDT.
- Provide nutritional supplements to participants as ordered by the Primary Care Physician and/or directed by the Dietitian.
- Work with the Dietitian to ensure participants receive diet-appropriate meals.
- Assist in monitoring food and fluid intake for specific participants as per their Plan of Care.
- Assist in monitoring participant physical and chemical restraints as per their Plan of Care.
- Ensure portable meals are sent home with participants as approved by the IDT.
- Assist participants upon arrival and departure from the PACE Center.
- Immediately report any observed changes in a participant's behavior, health, or mobility to the Center Coordinator and Nurse.
- Participate on the Interdisciplinary Team as required and communicate effectively with participants, family members, caregivers, staff, and visitors.
- Monitor safety, infection, and exposure control, and be prepared in case of an emergency.
- Complete documentation in the medical record accurately and in a timely manner.
- Comply with safety policies and procedures, identifying and reporting any potential or actual unsafe acts or conditions to the Center Coordinator and/or Center Manager.
- Meet or exceed BCSC targets for productivity.
- Continually seek better ways to deliver services and communicate with participants.
- Meet or exceed BCSC customer service targets.
- Demonstrate respect for and promote participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
- Understand the importance of community involvement and participate in activities that link BCSC to its communities.
- Effectively collaborate with staff peers and contractors to meet BCSC goals and further success.
- Comply with all policies and procedures of BCSC.
- Demonstrate proficiency in delivering age-specific care.
- Meet or exceed BCSC quality assessment and performance improvement targets.
Requirements:
- High school diploma or GED.
- Certified Nurse Assistant (CNA) certification.
- Verification of completion of CPR (BLS) and first aid training.
- Valid California driver's license with a good driving record.
- TB screening and successful completion of a health examination by a hospital-designated nurse practitioner or physician.
- At least one year of experience as a home aide or personal care attendant.
- At least one year of experience working with the frail or elderly.
- Ability to provide care for the frail elderly.
- Teamwork and problem-solving skills.
