Foster Care Services Coordinator

As a Foster Care Services Coordinator with UCAN, you will play a vital role in providing direct individual and family casework services to facilitate healing through trauma-informed services and positive youth development. Our team works collaboratively to ensure the best possible outcomes for our clients, and we are seeking a skilled professional to join us in this endeavor.
Key Responsibilities:
• Assessment and Service Planning: Conduct comprehensive assessments and develop service plans that meet the unique needs of our clients and their families. Implement and monitor these plans, ensuring progress and making necessary recommendations and referrals as required.
• Organization and Service Delivery: Develop a working knowledge of community resources to effectively serve our clients and maintain strong relationships with these resources. Provide referrals and linkages to appropriate services, securing written agreements as needed. Recommend and obtain supervisor approval for limited financial assistance to support client needs.
• Documentation and Reporting: Maintain accurate, complete, and timely client records and data, adhering to UCAN and IDCFS requirements. Prepare and submit reports to supervisors, management, and DCFS as required, ensuring clear and concise communication. Report all critical incidents and unusual events to the supervisor, management, or DCFS as mandated by law.
• Client Outreach and Engagement: Engage in regular face-to-face contact with clients through home and office visits, maintaining a required number of visits. Develop effective helping relationships with clients and their families, and provide outreach to significant people in the client's life to maintain contact and develop resources for the client.
• Support Parenting Skills and Child Development: Conduct parenting assessments and child development screens for assigned clients and their child(ren), providing individualized education to teen parents regarding developmental expectations, health and safety issues, and infant/child bonding. Assist clients in understanding and following through with recommendations made by their children's service providers when necessary.
• Professionalism and Work Conduct: Demonstrate integrity, high ethical standards, and a positive image of UCAN at all times. Act in a professional manner, maintaining appropriate boundaries with clients and demonstrating accountability for results and commitments to others.
Requirements:
• SW or BS/BA in a Human Service field is required, along with licensure as a Child Welfare Services Employee or eligibility for licensure. 0-2 years of experience working in human services, child welfare, or crisis intervention is also necessary for this role.
• Estimated Salary: $55,000 - $65,000 annually, depending on experience and qualifications.
