Job Title: Wellness Nurse

We are seeking a skilled and compassionate Wellness Nurse to join our team at Inspir Carnegie Hill. As a key member of our Integrated Care team, you will play a vital role in delivering high-quality nursing care to our residents.

Job Summary:

The Wellness Nurse will be responsible for providing nursing care to residents, supervising the care delivered by Care Partners, and managing medications. You will work closely with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities:
  • Provide outstanding customer service to residents, family members, and visitors
  • Coordinate and deliver nursing care to residents, including catheter care, wound care, and medication management
  • Supervise the care provided by HHAs and RCAs to residents
  • Manage and administer medications, and assist residents with self-administration
  • Document care and services provided to residents in the electronic health record
  • Participate in health, wellness, and lifestyle programs, and assist with all aspects of resident care and experience
Requirements:
  • Must be a Licensed Practical Nurse, licensed to practice in the state of New York
  • Minimum of two years of full-time clinical experience in nursing, with at least one year in a home health care agency, hospital, skilled nursing facility, or community health program
  • Basic first aid and CPR certification
  • Experience in assisted living, post-acute, or long-term care setting preferred
  • Ability to work well in stressful, high-pressure situations, and maintain composure and objectivity under pressure
