Job Summary:We are looking for a highly motivated and experienced LPN to coordinate our Diabetes and Chronic Disease Management Program. The successful candidate will be responsible for ensuring high adherence rates to the program, providing education to employees, and establishing strong relationships with patients.
Responsibilities:- Coordinate between patient, clinical, and operations partners to set up appointments, follow up, and gather feedback.
- Ensure high adherence rates to the program in accordance with our guidelines.
- Provide education on diabetes management and exercise advice to help clients establish healthy eating habits and lifestyle.
- Support clients in diabetes management and other aspects of well-being and lifestyle.
- Establish and maintain rapport and good relationships with patients.
- Maintain client documentation.
- Bachelor's degree in nursing, health administration, or a related field.
- Active license of Licensed Practical Nurse (LPN) in NY or NJ (preferred).
- Three to five years of experience in nursing, health administration, or a related field.
- Experience working with diabetes patients. Certified Diabetes Educator strongly preferred.
- Experience with vaccine and phlebotomy strongly preferred.
- Proficient with computers, Microsoft Office, EMR systems.
- Customer service-oriented and have great bedside manner.
- Strong communication skills; both written and verbal.
- Ability to work independently with little supervision.
- Bilingual with Haitian Creole or Spanish speaking strongly preferred.
- Must be able to reliably commute throughout NYC and/or NJ.
