Care Manager RN Job Description

ConnextCare is seeking a skilled RN Care Manager to join our team in Oswego, NY. As a key member of our care management team, you will be responsible for coordinating care for patients with complex medical and behavioral health needs.

Key Responsibilities:

  • Develop and implement care plans to improve patient outcomes
  • Collaborate with healthcare providers to ensure timely and effective care
  • Provide education and support to patients and families
  • Monitor patient progress and make adjustments to care plans as needed

Requirements:

  • Graduate of an accredited RN nursing program
  • Current New York State license in Registered Nursing
  • Experience in care management or a related field
  • Strong communication and interpersonal skills

Salary: $66,820.00-$78,520.00/year depending on experience
