Job Summary:
Alternate Solutions Health Network LLC is seeking a skilled Speech Language Pathologist to provide home-based skilled speech therapy care for patients. The ideal candidate will have a current license in the state of employment and a minimum of one year of experience as a Speech Therapist in an acute care setting. Home care experience is preferred.
Responsibilities:
Develop an individualized ST plan of care and make adjustments as needs change.
Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.
Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
Requirements:
Current license in the state of employment.
Minimum one year of experience as a Speech Therapist in an acute care setting. Home care experience preferred.
Valid driver's license and auto insurance in your name as a driver.
Capable of all physical demands.
