We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary care team, you will provide high-quality, patient-centered care in the comfort of patients' homes.

About the Role

The successful candidate will have a current license to practice physical therapy in the state of employment and a minimum of one year of experience as an PT in an acute setting. Home care experience is highly desirable. The ideal candidate will be able to effectively communicate with patients, families, physicians, and coworkers, possess knowledge of therapy best practices, and demonstrate strong critical thinking skills.

Key Responsibilities
  • Provide comprehensive skilled services as ordered by the attending physician, including educating patients and their family members on disease management and prevention.
  • Develop individualized plans of care based on patient needs and progress, ensuring seamless continuity of care throughout treatment.
  • Collaborate with the interdisciplinary team to ensure timely and effective communication, achieving optimal patient outcomes.
  • Maintain accurate and detailed records of patient interactions, treatment plans, and progress, adhering to agency protocol and regulatory guidelines.
Benefits

We offer a competitive compensation package, including a salary range of $75,000 - $100,000 per annum, depending on experience. Location

This position is based in the Cincinnati, OH area, serving Hamilton, Butler, Warren, Clermont, Clinton, Highland, and Brown counties. Travel may be required to visit patients in their homes.
