Position Summary

Under general supervision, the Healthcare Support Specialist is dedicated to the care and development of individuals within one of the agency's residential facilities. This role involves interaction that promotes dignity and learning, ultimately leading to enhanced independence.

Key Responsibilities:

  • Schedule medical appointments for residents.
  • Arrange and/or provide transportation for individuals to their medical appointments.
  • After medical visits, verify if the resident has a medication order, new prescriptions, or follow-up appointments.
  • Ensure medications are ordered, documented accurately, and managed as prescribed.
  • Conduct bi-weekly checks on medications and medical supplies to maintain adequate stock for the medication program.
  • Maintain resident medical records in compliance with agency policies.
  • Document all activities in the electronic medical record system upon completion of training.
  • Assist patients to and from examination rooms and prepare them for clinician consultations.
  • Communicate relevant clinical findings to nursing staff or clinicians as necessary.
  • Perform Direct Care duties as required.

Qualifications

Educational Requirements:

  • High School Diploma or GED

Required Skills:

  • Valid Driver's License (Required)
  • Must possess a current and clean driver's license.
  • Previous experience as a Direct Support Professional, Nursing Assistant, or Patient Care Assistant is advantageous.
  • Highly organized, capable of working under pressure, and adept at multitasking.
  • Maintain professionalism at all times.
  • Familiarity with OPWDD policies and all relevant state and federal regulations regarding individual care and treatment.
  • Understanding of active treatment programs related to individual needs.
  • AMAP certification or ability to obtain certification within six months of hire.
  • SCIP certification or ability to obtain certification within six months of hire.
  • CPR and First Aid certification or ability to obtain certification within six months of hire.
  • Must be fully vaccinated against COVID-19.

Shift Availability: Full-Time, Monday – Friday 6am – 2pm

Available Work Locations: Bronx, New York and Westchester County
