Position Overview:

As a dedicated Hospice RN Case Manager, you will deliver exceptional nursing care to hospice clients in accordance with physician directives and family requests, while adhering to state regulations and organizational policies. You will take charge of client assessments, care planning, therapeutic interventions, and the overall management of client care and outcomes, serving as the primary case manager for hospice patients.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans for hospice patients.
  • Collaborate with physicians and interdisciplinary teams to effectively manage patient symptoms.
  • Provide guidance and support to Hospice Aides and LPN/LVNs as necessary.
  • Engage with families to address grief and loss issues, ensuring compassionate support throughout the care process.
  • Participate in weekly interdisciplinary meetings to discuss patient care strategies.

Territory: Athens, GA

Qualifications:
  • Graduation from an accredited nursing program.
  • Current Registered Nurse licensure in the State of Georgia.
  • A minimum of one year of experience in acute care nursing.
  • Experience in home care is preferred but not mandatory.
