Job Summary
We are seeking a highly skilled Care Manager (Registered Nurse) to join our team. This individual will be responsible for patient care coordination from admission through discharge, ensuring smooth transitions of care and maximizing reimbursement. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.

About the Role
The Care Manager will begin the process of care coordination at the time of the patient's admission by completing a thorough admission assessment and/or psychosocial assessment. This individual will work with the hospital care team and the patient/family to plan and implement the best possible plan for the patient, taking various factors and limitations into consideration. The Care Manager will also identify and recommend post-acute services, complete referrals to appropriate providers, and ensure the discharge plan is aligned with the patient's medically cleared date and projected length of stay.

Requirements
To be successful in this role, you will need:
• A valid, active unencumbered Nursing license
• Bachelors degree in nursing or actively being pursued
• 2 years recent healthcare experience
• Ability to meet quality and productivity expectations and complete yearly competencies

Salary Information
This role offers a competitive salary range of $44.00 - $50.60 per hour, based on location and experience. We offer a comprehensive benefits package, including health insurance, retirement plans, and paid time off.
