Nurse Navigator Specialist

At Insight Global Healthcare, we are seeking a highly skilled Nurse Navigator Specialist to join our team. As a key member of our Medical Command Center, you will play a vital role in providing exceptional patient care and navigation services across the country.
**Job Summary:**
The Nurse Navigator Specialist will be responsible for triaging patients over the phone, navigating their care to appropriate facilities, and conducting remote telemonitoring for patients nationwide. This is a dynamic and fast-paced role that requires strong clinical skills, excellent communication abilities, and the ability to multitask and adapt to change.
**Responsibilities:**
- Triage patients according to established guidelines over the phone
- Navigate care to appropriate facilities across the country
- Conduct remote telemonitoring for patients nationwide
- Participate in Quality Improvement activities
**Requirements:**
To be successful in this role, you must have:
- An active LVN or RN license without restriction (must be willing and able to achieve licensure in other states and countries)
- Graduation from an accredited nursing school
- Experience with telephonic assessing
- Experience with remote patient monitoring
- Experience working with all patient ages, from pediatric to geriatric
- Knowledge of clinical practices, procedures, and protocols
- Ability to multitask and adapt to change
- Strong computer skills, including Microsoft Word and Excel
- Excellent phone presence and communication skills
**Estimated Salary Range:** $65,000 - $85,000 per year, depending on experience and location.
