Job Summary

We are seeking a skilled Medical Social Worker to join our team at MorseLife Health System. As a Medical Social Worker, you will provide essential support to terminally ill individuals and their families, working collaboratively with our transdisciplinary team in various settings.

Key Responsibilities
  • Address the emotional, spiritual, and psychosocial needs of patients and families related to their illness, care needs, treatment response, and adjustment to care, in accordance with the Team's Transdisciplinary Plan.
  • Assist with group support programs for patients and families/caregivers, promoting a sense of community and connection.
  • Maintain and promptly complete all required records and reports, adhering to agency standards and ensuring accurate documentation.
  • Participate in care conferences and bereavement programs, providing a supportive and empathetic presence.
  • Assist with discharge planning, ensuring a smooth transition for patients and families.
  • Refer patients and family members to community resources, facilitating access to necessary support services.
  • Exhibit flexibility in responding to emergency situations, demonstrating adaptability and a commitment to patient care.
  • Demonstrate positive customer service responsiveness, upholding agency standards and promoting a culture of excellence.
  • Be flexible in meeting patient/family needs, including working evenings and weekends, as required.
  • Participate in the on-call rotation, as scheduled, ensuring 24/7 coverage and support.
  • Assist in preparing financial assessments and payment agreements, ensuring accurate and timely documentation.
  • Participate in chart review, quality improvement activities, and other meetings as assigned, contributing to a culture of continuous learning and improvement.
  • Complete annual competency requirements, maintaining expertise and staying up-to-date with industry developments.
  • Assist with the completion of Medicaid application processes, ensuring patients and families receive necessary support.
  • Assist with the orientation of new employees, promoting a culture of onboarding and integration.
  • Participate in in-service programs and outside public relations activities, as requested, fostering a sense of community and collaboration.
  • Serve on committees as assigned, contributing to the development of policies and procedures.

Qualifications

  • Masters prepared Social Worker.
  • Experience (2 yrs.) in social work or related work with individuals, families, and groups.
  • Experience preferred in hospice, medical facilities, or nursing homes.
  • Knowledge of community resources.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
