Job Title: Social Worker

We are seeking a skilled Social Worker to provide medical social services to patients in their homes, under the direction and supervision of our Clinical Manager.

Responsibilities:
  • Conduct initial assessments of patients and families to determine home care needs, including physical, emotional, and social factors.
  • Develop and implement individualized care plans tailored to each patient's needs and goals.
  • Assist in the admission process, performing evaluations and assessing socioeconomic and emotional factors that affect treatment.
  • Consult with attending physicians regarding plan of treatment alterations.
  • Maintain and submit written clinical records, adhering to established Agency policies and procedures.
Requirements:
  • Master's Degree from a School of Social Work, approved by the Council of Social Work Education.
  • Must have, or be in the process of acquiring, certification from the Academy of Certified Social Workers.
  • Two years' experience preferred, with at least one year in a healthcare setting.
  • Must have a criminal Level II background clearance and current CPR certification.
