Job Summary

We are seeking a highly skilled and motivated Population Health Outreach Specialist to join our team at Blue Cross and Blue Shield of Louisiana. As a key member of our Population Health team, you will play a critical role in coordinating population health activities for members, providing administrative support, and assisting clinical staff with discharge planning and coordination.

Key Responsibilities
  • Coordinate population health activities for members via inbound and outbound calls to engage, educate, and transfer for enrollment in programs and/or reconnect those lost to contact.
  • Assist clinical staff with discharge planning and coordination, creating episodes/activities in care management systems, arranging services, and working reports as needed.
  • Provide administrative support, including accurate and timely program data entry, responding to benefit inquiries, and completing care coordination activities delegated by clinical staff.
  • Support clinical teams by researching current and new resource capabilities to identify efficiencies and quality solutions for fulfillment, documentation, and member interactions.
  • Handle routine telephone calls, visitors, and incoming and outgoing correspondence received by the Population Health teams and make outbound telephone calls based on the need of targeted populations.
Requirements
  • High School Diploma or equivalent required; two years of college preferred.
  • Three years of general health insurance, managed care experience, and/or prior customer service required.
  • One year of experience communicating with members about clinical topics required.
  • Four years of work experience where duties include problem-solving and customer satisfaction preferred.
  • Clinical office experience preferred.
Preferred Qualifications
  • Certification in a medical-related field preferred.
  • Working knowledge of relevant software and proficiency in Microsoft Word and Excel required.
