We are seeking a highly skilled Clinical Pharmacist to join our team in the Emergency Department/Critical Care unit at Sutter Health. As a Clinical Pharmacist, you will play a critical role in providing patient-centered care, medication information, education, and medication preparation and distribution to ensure safe, effective, and cost-efficient medication therapy.
Key Responsibilities- Deliver pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution.
- Exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner.
- Precept Pharmacy Residents in an accredited Residency Program.
- Gain confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication.
- Adhere to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.
- Doctorate: Graduation from an accredited school of Pharmacy.
- PHARMD-Licensed Pharmacist or PHARMR-Registered Pharmacist.
- 3 years of clinical and practical experience as typically acquired in a similar environment.
- 3 years of residency training or equivalent clinical experience.
- Ability to influence physician prescribing at the service-line level.
- Demonstrated skills in managing projects and initiatives which have impact at the facility level.
- Must meet qualifications to precept Pharmacy Residents in an ASHP accredited Residency Program.
This position may regularly work, store, prepare, receive, unpack, transport, dispose of, or administer drug(s) identified as hazardous, or potentially hazardous, by the National Institute for Occupational Safety and Health (NIOSH) for purposes of USP 800.
