Job Description

The City and County of San Francisco is seeking a highly skilled Clinical Pharmacist to join its team. As a key member of the Department of Public Health, this individual will play a critical role in ensuring the health and well-being of the community.

Key Responsibilities
  • Conduct patient interviews and examinations to evaluate drug usage and adherence.
  • Consult with physicians to discuss drug usage and recommend changes to medication regimens.
  • Develop and implement training programs for healthcare staff on drug utilization and dosage.
  • Participate in the development of medication use policies and guidelines.
  • Compound and dispense medications, manage pharmacy stock, and maintain accurate records.
Requirements
  • Valid Registered Pharmacist license issued by the California State Board of Pharmacy.
  • One of the following: completion of a pharmacy residency program, completion of a post-graduate pharmacy fellowship program, two years of professional level experience as a Clinical Pharmacist, or possession of a Board Certification in a pharmacy specialty area.
Preferred Qualifications
  • Acute Care practice: demonstrated experience with the acute care medicine patient population, clinical and/or lived experience with diverse communities, excellent oral and written communication and interpersonal skills.
  • Ambulatory Care practice: completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship, fluency in Spanish and/or Cantonese, mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.
  • Behavioral Health practice: completion of a psychiatric pharmacy residency, clinical experience working in substance use disorder or mental health settings, fluency in Spanish and/or Cantonese, mission-driven, passionate candidates with Mental Health/Behavioral Health/Substance Use clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.
  • Long Term Care practice: completion of a geriatric pharmacy residency, certification in Geriatric Pharmacy.
Selection Procedures

Applicants will be required to complete a Minimum Qualification Supplemental Questionnaire (MQSQ) and a Training and Experience Evaluation Supplemental Questionnaire. Candidates deemed qualified will be placed on an eligible list and given a score and a rank.
