Masonic Village Home Health, Hospice and Home Care is seeking a skilled RN Hospice Case Manager to join our team. As a RN Hospice Case Manager, you will be responsible for ensuring quality care of our Hospice Admissions in a timely and appropriate manner. You will work closely with our interdisciplinary team to provide comprehensive care to our patients and their families.
Key Responsibilities:- Maintain a working knowledge of standards of care for hospice programs to ensure compliance with Medicare certification and state licensure.
- Complete documentation, including assessments, certifications, and care plans, in accordance with Medicare Conditions of Participation for hospice programs.
- Attend and participate in IDG, mandatory staff meetings, and in-service programs.
- Communicate with patients, families, primary care physicians, and facility staff to ensure seamless care coordination.
- Monitor quality end-of-life care by assessing patient needs and coordinating care with other members of the IDG.
- Provide education to patients, families, caregivers, and friends on end-of-life care and respond to their physical, mental, social, and emotional needs.
- Current RN licensure by the Pennsylvania State Board of Nursing.
- CPR certification prior to or upon employment.
- Previous hospice experience.
- Valid PA driver's license and insurance on personal vehicle.
