Job Summary

We are seeking a skilled Mental Health Case Manager to join our team at Service Access & Management, Inc. As a key member of our Mental Health Team, you will provide Blended Case Management services to individuals in need of support.

Key Responsibilities:

  • Provide Blended Case Management services to individuals, including assessment, goal setting, and service coordination.
  • Build and maintain relationships with individuals, families, and providers to ensure effective service delivery.
  • Collaborate with team members to support the people we serve and achieve our team's mission.
  • Facilitate meetings and discussions to promote effective communication and problem-solving.
  • Advocate for individuals and families to ensure their needs are met and their rights are respected.
  • Think critically and problem-solve to address complex issues and challenges.
  • Provide crisis intervention and support to individuals in crisis.
  • Link individuals to community resources and services to promote independence and self-sufficiency.
  • Assess needs and develop Individualized Service Plans to meet the unique needs of each individual.
  • Complete and update case notes and documentation to ensure accurate and timely record-keeping.

Requirements:

  • Bachelor's Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field.
  • Or, 12 credit hours in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field AND 2 years experience.
  • MH/DD experience preferred.
  • Automobile and Valid Insurance, Valid Pennsylvania Driver's License.

Working Conditions:

  • Prolonged periods working on a computer in an open office environment.
  • Comfortable communicating through virtual technology.
  • Work may be required at nights and on weekends.
  • Travel throughout the region.
  • Travel may be in adverse weather conditions.
  • Must be able to lift to 15 pounds.
