About the RoleWe are looking for a dedicated professional to provide integrated and coordinated case management services for high-risk and high-utilizing populations in both inpatient and outpatient psychiatry and addiction medicine settings.
Key Responsibilities- Initial Intervention: Engage with high-risk, high-utilizing patient populations and act as a central point of contact for referrals from multiple departments.
- Assessment and Evaluation: Provide assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities, and networking skills.
- Planning of Services: Develop plans based on assessments of both short and long-term needs.
- Tracking and Quality Management: Track referrals and outcomes for ongoing quality management.
- Consultation and Coordination: Operate as a consultant for other departments and facilitate collaboration between Primary Care, Behavioral Health Care Services, and Health Care Providers.
- Experience: Minimum two years recent experience in case management and working with community resources.
- Education: Master's degree in Social Work, Psychology, Sociology, or other field related to behavioral health.
- License: Licensed Professional Clinical Counselor (California) or Licensed Marriage and Family Therapist (California) or Licensed Clinical Social Worker (California).
