Job Summary

We are seeking a highly skilled and experienced Staff Pharmacist to join our team at KPH Healthcare Services. As a key member of our clinical operations team, you will play a critical role in ensuring the efficient and effective delivery of high-quality patient care.

Key Responsibilities
  • Compliance and Regulatory Management: Ensure compliance with federal, state, and local laws and regulations, as well as accrediting body standards.
  • Medication Order Review and Management: Interpret and evaluate physician medication orders to ensure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness.
  • Record Keeping and Documentation: Accurately maintain all legally required records and documentation.
  • Supervision and Training: Directly supervise pharmacy support personnel and/or interns in the preparation, packaging, and labeling of pharmaceuticals.
  • Software and Technology: Proficiently use CPR+ software and other relevant technology to support clinical operations.
  • Training and Development: Assist in the training and orientation of new employees and participate in ongoing professional development.
  • Communication and Collaboration: Provide technical and clinical information to patients, nurses, and physicians, and communicate results of monitoring activities as required.
  • Quality Control and Assurance: Initiate, review, and update all required documentation, and adhere to stability and compatibility guidelines and quality control procedures.
  • Inventory Management: Follow established procedures for DME and supply procurement, and maintain appropriate inventory levels and preferred product selection.
  • Compounding and Dispensing: Accurately compound IV admixtures, utilizing appropriate technique, performing checks and double checks.
  • Security and Compliance: Maintain procedures for the secure storage and handling of legend items, including controlled substances.
  • Order Entry and Labeling: Accurately enter and label medication orders.
  • Clinical Activities: Participate in clinical activities, including start of care prospective review of medical and drug histories, pharmacokinetic dosing, dosage monitoring for compromised patients, reviewing laboratory test results, attending patient care conferences, and developing and updating patient-specific plans of care.
  • Referral and Product Selection: Participate in the referral acceptance/denial process and product, device, and supply selection.
  • On-Call Rotations: Participate in on-call rotations as required.
  • Performance Improvement: Participate in Performance Improvement activities, educational programs, and departmental/branch meetings.
  • Case Management: Drive the Primary Case Management Model (PCMM) in daily interactions through delegation of non-cognitive activities, ordering clinically necessary and cost-effective labs, and facilitating positive patient outcomes with patients, nursing agencies, and other healthcare providers.
Qualifications
  • Education: Bachelor's Degree (BS) in Pharmacy or a Pharm.D. from an accredited college or university.
  • Experience: Prefer one year of experience in an IV alternate site environment or equivalent work experience.
  • Credentials/Licensure: Current license for state of practice, current CPR certification if involved with direct patient contact, and valid driver's license and appropriate automobile insurance coverage per AristaCare policy.
  • Special Conditions of Employment: Drug test, initial and continuous exclusion and sanction/disciplinary monitoring, and any additional eligibility requirements based on the specific position.
  • Required Training: HIPAA Privacy Course and HIPAA Security Course.
  • Job Skill Requirements: Ability to read, analyze, interpret general business periodicals, professional journals, technical procedures, or governmental regulations; ability to write reports, business correspondence, and procedure manuals; ability to effectively present information and respond to questions from groups of managers, patients, nurses, and physicians; ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, percent, and to draw and interpret graphs; ability to solve word problems and algebraic equations; and ability to make accurate pharmaceutical calculations.
