Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Oklahoma Heart Hospital. As a key member of our healthcare team, you will play a vital role in developing and implementing individualized diabetes management plans to reduce cardiovascular risk in patients living with diabetes.

Key Responsibilities
  • Collaborate with referring cardiologists and physicians to develop comprehensive diabetes medication management plans, educational goals, and teaching methods.
  • Provide general medication assessment and management based on current nationally recognized guidelines.
  • Assess and manage medication therapy for comorbidities that increase cardiovascular risk, including lipids, blood pressure, and renal protection.
  • Develop and communicate individualized follow-up plans with each patient, documenting sessions via electronic medical records in a timely manner.
  • Evaluate the effectiveness of management and educational services by measuring attainment of patient-specific goals.
  • Participate in the development of training materials and contribute to a continuous quality improvement process.
Requirements
  • Requires a bachelor's or PharmD degree from an accredited school of pharmacy.
  • American Society of Health-System Pharmacists Pharmacy Practice Residency preferred.
  • Priority will be given to a PharmD with certification in diabetes education or clinical diabetes medication management experience.
  • Minimum 6 months of experience providing diabetes management and 15 contact hours within the past year in diabetes clinical and educational subjects is preferred.
