We are seeking a highly skilled and compassionate Transplant Coordinator to join our team at West Virginia University Hospital, Inc. As a key member of our transplant team, you will play a vital role in ensuring the smooth coordination of patient care, from initial evaluation to post-transplant follow-up.
Key Responsibilities- Coordinate transplant evaluations, including reviewing patient records, identifying needs for testing, and communicating with the transplant team.
- Provide education to patients and families on transplant evaluation, OPTN regulations, organ allocation, and post-transplant care.
- Track testing and consults, ensuring all necessary information is obtained and communicated to the transplant team.
- Collaborate with multidisciplinary teams, community resources, and agencies to ensure comprehensive patient care.
- Participate in patient selection committee and advocate for patients' needs.
- Document all interactions and findings in the EMR, providing data to referring MDs, insurance companies, and regulatory agencies.
- Current unencumbered licensure with the WV Board of Registered Professional Nurses or appropriate state board.
- Obtain certification in Basic Life Support within 30 days of hire date.
- Three years of nursing clinical experience.
- Bachelor's degree in Nursing (preferred).
This role requires strong interpersonal and communication skills, proficiency in computer/keyboard skills, and the ability to work independently and handle multiple tasks. The physical demands of this position include full range of body motion, manual finger dexterity, and hand and eye coordination. The work environment is fast-paced and requires working under stressful conditions and irregular hours.
