Cancer Care Coordinator Role at WVU Medicine

Welcome to WVU Medicine, where we're dedicated to providing exceptional care to our patients and their families. We're excited to announce a new opportunity for a Cancer Care Coordinator to join our team.

As a Cancer Care Coordinator, you will play a vital role in ensuring that our patients receive comprehensive and coordinated care throughout their cancer journey. Your primary responsibility will be to coordinate and monitor all phases of cancer services, including prevention, screening, diagnosis, treatment, survivorship, death, and dying for patients and caregivers.

Key Responsibilities:

  • Coordinate and monitor all phases of cancer services, including prevention, screening, diagnosis, treatment, survivorship, death, and dying for patients and caregivers.
  • Act as a liaison between patients, physicians, and the hospital in transitioning the patient throughout the healthcare system, including inpatient and outpatient environments.
  • Collaborate with financial counselors and social services to ensure completion of financial assessments.
  • Collaborate with managers and agencies regarding outlined plans of care, assist with referrals, problem-solving, or overcoming obstacles to enable patients to achieve optimal levels of functioning.
  • Routinely monitor service schedules, adjust appropriately to maximize effective and efficient patient flow through intradepartmental lab, clinic, or infusion center.
  • Attend inpatient/discharge rounds, tumor boards, etc., as appropriate to obtain necessary information for communication with other departments/agencies regarding patient status and healthcare needs.
  • Coordinate long-term follow-up of patients with their local/referring physicians.
  • Maintain a retrievable patient list that includes key information such as name, DOB, patient status, treatment plan status.
  • Maintain written/verbal communication with referring institutions and physicians.
  • Assist with completion of healthcare-related paperwork (i.e., Leave of absence, FMLA, return to work/school, etc.).
  • Apply nursing process to each patient and/or family based on experience, theoretical knowledge, and clinical expertise.
  • Provide initial teaching to patients and caregivers, perform ongoing teaching to complex/high-risk patients and caregivers, delegate and coordinate teaching of intermediate or low-risk patients and caregivers as appropriate.
  • Evaluate the effectiveness of teaching efforts and modify education plans as needed.
  • Develop service-specific patient/caregiver education materials.
  • Coordinate and/or participate in appropriate responses to life-threatening emergencies.
  • May provide hands-on patient care in the lab, clinic, or infusion center by performing nursing assessments, chaperoning, assisting with or performing procedures, phlebotomy, IV therapy, and other direct care as appropriate to keep patients flowing through service areas. Triage incoming patient telephone calls. Perform proactive outgoing telephone triage by routinely contacting high-risk patients and caregivers to check on health status and service needs.
  • Demonstrate accountability for nursing practice and identify own learning needs.
  • Assist the Director/Manager and/or multidisciplinary team in quality improvement activities and integrate information gained through quality improvement activities and clinical nursing research into practices.
  • Develop, revise, and follow established standards of care, standards of practice, and policies and procedures.
  • Participate in clinical research activities.
  • Provide feedback as requested for evaluating employee performance.
  • Assist in the orientation of new employees.

Requirements:

  • Licensure to practice as a registered nurse in West Virginia.
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Oncology Certified Nurse (OCN) or Blood and Marrow Transplant Certified Nurse Certification (BMTCN) required within two (2) years of hire.
  • Three (3) years nursing experience.
  • Prior experience with hematology/oncology.
  • Five (5) years of clinical nursing experience.
