Clinical Case Manager

Job Summary:
The Clinical Case Manager will work as part of a multidisciplinary team to provide comprehensive and integrated treatment services to tenants of Rene Cazenave and Richardson Apartments. The ideal candidate will have experience in case management and mental health treatment for at-risk adults and/or older adults, including co-occurring disorders.
Key Responsibilities:
- Evaluate the needs of clients and develop treatment plans, often in collaboration with the onsite medication support team.
- Work closely with an interdisciplinary team to provide onsite support to residents in the RCA and Richardson apartments.
- Create a welcoming community milieu within the apartment building by supporting meal programs, community building events, and tenant organizing.
- Collaborate with property management to support tenants in maintaining stable independent housing.
- Participate in Officer of the Day rotation to manage crises, drop-ins, and wellness checks.
- Perform intakes and comprehensive psychiatric assessments for adults and older adults with psychiatric and dual diagnoses.
- Provide crisis intervention and management to prevent eviction and hospitalization, and if necessary, facilitate involuntary hospitalization.
- Provide individual and group therapy to stabilize clients and prevent psychiatric de-compensation.
- Provide case management services, including evaluations, service plan development, and assisting clients in accessing housing, financial, medical, and other needed services.
- Coordinate with other service providers.
- Maintain thorough clinical records in accordance with mandated standards of San Francisco Community Mental Health Services and funders.
- Participate in team meetings and case conferences through presentation of cases, discussion of clinical issues, and exchange of information and ideas.
Requirements:
- Master's degree or higher in Social Work or other Professional Counseling Programs, and registered with an active license with BBS.
- Experience in case management and mental health treatment for at-risk adults and/or older adults, including co-occurring disorders.
- Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care Services definitions.
- Excellent communication skills in written and verbal formats.
- Knowledgeable about and consultation-oriented towards client risks for suicidality/violence, abuse reporting, dual role conflicts, and other ethical/legal obligations.
- Solution-focused care planning and conceptualization skills within a multi-disciplinary team and with external agency partners and individuals.
- Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds.
- Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5-TR and other measures.
- Ability to work well under pressure while meeting multiple different types of deadlines.
- Computer skills proficiency in applications for electronic health care records, Zoom, Box, Microsoft/Outlook applications, and other technologies as needed for the role.
