Case Manager II, Contract Lead

Job Summary:
The Case Manager II, Contract Lead will serve as a liaison between Goodwill and CCB to implement a case management program for assigned consumers in ABILITY ONE. The incumbent will provide intensive support to achieve individual goals and maximize independent functioning in a work and community environment.
Key Responsibilities:
- Provide direction, supervision, and case management for assigned consumers.
- Develop and implement individual program plans, documenting progress for clients.
- Plan ancillary activities, including special staffing and referrals for special services.
- Encourage independence by assisting consumers in problem-solving personal and work-related issues.
- Teach proper work habits, interaction skills, and self-help skills through role modeling and programmatic intervention.
- Monitor consumer production and facilitate training in production methods to increase productivity.
- Attend annual staffing-funded clients and provide crisis intervention when necessary.
Requirements:
- Bachelor's degree in Vocational Rehabilitation, Psychology, Sociology, Counseling, or related field.
- Applicable field experience or equivalent combination of training and experience in social services, counseling, or rehabilitation settings.
- Excellent written and verbal communication skills, with ability to interact with various types of internal and external customers.
- Ability to relate effectively to disabled persons, their parents, and guardians, and be empathetic toward their needs.
