Case Manager II, Contract Lead

The Case Manager II, Contract Lead is responsible for providing intensive support to consumers assigned to the ABILITY ONE program. The incumbent will develop and implement individual program plans, document progress, and maintain accurate records. The position requires excellent written and verbal communication skills, as well as the ability to interact with various types of internal and external customers.
Key Responsibilities- Develop and implement individual program plans for assigned consumers
- Document progress and maintain accurate records
- Provide direction, supervision, and case management for assigned consumers
- Collaborate with other involved agencies and support personnel to ensure comprehensive support for consumers
- Bachelor's degree in Vocational Rehabilitation, Psychology, Sociology, Counseling, or related field
- Applicable field experience or equivalent combination of training and experience
- Excellent written and verbal communication skills
- Ability to interact with various types of internal and external customers
