We are seeking a dedicated Personal Care Assistant to deliver daily living support and personal care services to clients in their residences, following a structured care plan. The position is available in various locations.
Key Responsibilities
- Support clients with Activities of Daily Living (ADLs) such as bathing, dressing, toileting, and mobility, adhering to the personalized service plan.
- Encourage clients to participate in their own care as much as possible.
- Prepare and serve meals to clients, adhering to a pre-established menu.
- Promote healthy eating habits and adequate hydration while respecting clients' choices, and report any changes in their dietary habits to the Care Manager.
- Engage clients in recreational activities and foster social interactions.
- Maintain accurate and timely records of client care in accordance with company policies.
- Document medication administration, leisure activities, incidents, and any observations, reporting changes in clients' physical or emotional states.
- Monitor and report on clients' health and emotional well-being, promptly communicating any changes to the Care Manager and family members.
- Assist clients with medication reminders.
- Ensure a clean, safe, and organized environment for clients, performing light housekeeping duties as needed.
- Occasionally transport and accompany clients to medical appointments.
- Provide companionship and basic emotional support to clients.
Qualifications and Skills
- Demonstrated experience as a licensed Home Health Aide.
- Current CPR certification.
- Clear background check; all criminal records, except minor traffic violations, require an exemption.
- Ability to monitor vital signs and collect samples.
- Understanding of basic nutrition and personal hygiene practices.
- Proficient in English.
- Skills in nursing and healthcare administration.
- Compassionate and caring demeanor.
- First Aid certification.
- Valid driver's license.
- High school diploma or equivalent.
