We are seeking a highly skilled Behavioral Health Clinician to join our team at CommUnityCare Health Centers. As an integral member of our care team, you will work closely with primary care providers, specialty providers, and medical residents to deliver high-quality behavioral health services to our patients.
Key Responsibilities- Provide assessments, screening, and intervention services to patients with behavioral health concerns.
- Collaborate with the care team to develop and implement individualized care plans.
- Consult with primary care providers and specialty providers to ensure comprehensive care.
- Deliver care using evidence-based treatments and best practices.
- Document patient care plans in the electronic health record.
- Participate in quality improvement activities to enhance patient care.
- Master's degree in Social Work, Counseling, Psychology, or related field.
- Current licensure as LCSW or LPC in the State of Texas.
- At least 2 years of clinical experience in mental health, with at least 1 year in a primary care setting.
- Experience working in a primary care setting.
- Knowledge of evidence-based treatments for mental and behavioral health conditions.
