Job Title: Master Social Worker - ESRD Care Specialist

At Innovative Renal Care, we are seeking a highly skilled Master Social Worker to join our team as an ESRD Care Specialist. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support.

Responsibilities:
  • Assess patients' physical, mental, and emotional needs, and develop personalized care plans to address their unique challenges.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.
  • Provide education and support to patients and their loved ones, empowering them to manage their ESRD and improve their overall quality of life.
  • Conduct thorough assessments, identify areas of concern, and develop strategies to address them, ensuring patients receive comprehensive care.

Requirements:
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current state licensure (e.g., LCSW, LMSW) and meet all state requirements for practice in a dialysis setting.
  • Two years of clinical experience in an ESRD setting, with a strong background in social work and patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
