Position Overview
The Patient Care Assistant plays a crucial role in delivering personal medical support and aiding with daily living tasks that enhance the well-being of our clients, enabling them to maintain their independence in a home or facility setting while upholding confidentiality and professional standards.

Key Responsibilities
As a Patient Care Assistant, your essential duties will include:
  • Administering, assisting, assessing, and documenting medical care, including:
    - Basic first aid
    - Monitoring weight and vital signs
    - Catheter and bowel management, including intake and output tracking
    - Conducting range of motion exercises and other activities as directed by therapy professionals
    - Supporting ambulation, transfers, and positioning using appropriate medical equipment
    - Preparing meals tailored to dietary and nutritional requirements
    - Addressing patients' specific conditions and needs within the authorized scope of practice
  • Assisting patients in maintaining personal hygiene, which includes:
    - Support with bathing, grooming, toileting, shaving, skin and nail care, oral hygiene, dressing, and eating
  • Ensuring thorough documentation and coordination of all necessary records, such as:
    - Completing patient and family documentation after each visit
    - Observing and reporting significant changes in the patient's condition to the nursing supervisor

Additional Duties
Provide hands-on personal care and perform basic procedures as an extension of nursing services, following the physician's care plan and adhering to state regulations.

Qualifications
To be considered for this role, candidates must possess:
  • A high school diploma or GED
  • Current, unrestricted certification or proof of training as mandated by state regulations
  • Valid CPR certification
  • Proficiency in clinical assessments and adherence to care policies and procedures
  • A valid driver's license and an acceptable motor vehicle record

State-Specific Requirements
Compliance with ongoing education and training as required by state regulations is essential.

Physical Requirements
