Job Summary

We are seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team at Stony Brook Medicine. As a key member of our cancer care team, you will play a vital role in ensuring that patients receive timely and effective care throughout their cancer journey.

Key Responsibilities
  • Provide personalized navigation and support to patients and their families, addressing their unique needs and concerns.
  • Facilitate communication between patients, healthcare providers, and other members of the care team to ensure seamless transitions and optimal care.
  • Identify and remove barriers to care, linking patients to resources and services that enable them to complete treatment and improve their quality of life.
  • Collaborate with the disease management team to develop and implement patient-centered care plans, ensuring that patients receive comprehensive and coordinated care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care and outcomes.
Requirements
  • NYS RN license
  • Five (5) years acute nursing experience, with minimum 3 years in oncology
  • Bachelor's degree in nursing
  • Preferred: Masters in Nursing, Certified Oncology Nurse, previous experience in case management, quality management, and/or patient education
