Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate with interdisciplinary teams to ensure seamless patient care
- Monitor patient progress and adjust care plans as needed
- Collaborate with physicians, nurses, and other healthcare professionals to ensure effective communication and coordination of care
- Identify opportunities for cost savings and resource optimization
- Develop and implement strategies to improve patient satisfaction and outcomes
Requirements:
- Graduate from an accredited school of vocational nursing
- Current license to practice as a Licensed Vocational Nurse in the State of Texas
- Three to five years of clinical experience, preferably in a hospital or acute care setting
- Knowledge of case management and utilization management principles
- Ability to communicate effectively with patients, families, and healthcare teams
