Responsibilities:
* Provide phone-based support to patients after discharge, addressing their concerns and ensuring a smooth transition to home care.
* Educate patients and caregivers on disease-specific education, empowering them to manage their health effectively.
* Coordinate follow-up care with primary care physicians or specialists as needed, ensuring timely and effective treatment.
* Identify barriers to compliance with the plan of care and develop strategies to overcome them.
* Refer patients to the Care Link Clinic when necessary, facilitating access to additional resources and support.
* Collaborate with healthcare professionals to coordinate access to clinical and community-based resources, promoting holistic patient care.
* Respond to triggers that may indicate the need for intervention, taking proactive steps to prevent complications.
* Maintain accurate records of problem status for follow-up, ensuring timely and effective resolution.
* Assist with meetings and disseminate information to relevant areas, as directed.
Requirements:
* Graduate of an accredited vocational nursing school.
* Current Texas licensure as a Licensed Vocational Nurse.
* Minimum of two years of clinical or related hospital experience.
* Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
* Demonstrated leadership experience and ability to work independently.
* Ability to read, write, and speak English required; ability to speak Spanish preferred.
