Job Summary

HealthONE Healthcare at Home is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care.
  • Collaborate with other healthcare professionals to ensure the provision of quality patient care.
  • Provide patient education and support to promote independence and well-being.
  • Document patient progress and communicate significant findings to the supervising RN and physician.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Current LPN licensure in the state of practice.
  • 1 year of experience in home health care or a related field.
