Key Responsibilities:
- Assist residents in achieving aftercare goals, including resolving pending legal matters and setting up medical and mental health providers
- Transport residents to and from outside appointments as needed
- Help navigate probation, parole, DCF, and other programs
- Collaborate with registered nurses and on-site psychiatric staff to provide comprehensive care
Requirements:
- Minimum of 1-3 years of experience in substance use and co-occurring disorders in an ASAM 3.1 level of care
- Prior knowledge and experience in addiction case management
- Strong communication and interpersonal skills
