As a Care Coordinator, you will play a vital role in assisting residents in achieving aftercare goals, including resolving pending legal matters, setting up medical and mental health providers, and navigating probation and parole programs.

Key Responsibilities:

  • Assist residents in achieving aftercare goals, including resolving pending legal matters and setting up medical and mental health providers
  • Transport residents to and from outside appointments as needed
  • Help navigate probation, parole, DCF, and other programs
  • Collaborate with registered nurses and on-site psychiatric staff to provide comprehensive care

Requirements:

  • Minimum of 1-3 years of experience in substance use and co-occurring disorders in an ASAM 3.1 level of care
  • Prior knowledge and experience in addiction case management
  • Strong communication and interpersonal skills
