Job Summary:

The Vascular Care Group is seeking a skilled Clinical Care Coordinator to join our team. As a key member of our healthcare team, the Clinical Care Coordinator will be responsible for providing exceptional patient care and advocating for optimal patient outcomes. This role will work closely with physician leadership and other nursing staff to ensure quality service for patients and maintain a cooperative and collaborative environment among all providers.

Responsibilities:

Oversee patient experience while in clinic
Advocate for optimal patient care and make recommendations as needed
Provide patient and caregiver education on patient care, treatment, and future prevention
Administer wound care per MD/NP orders
Ensure medical records are comprehensive, accurate, and follow HIPAA guidelines
Assess vital signs and use clinical judgment and nursing assessment
Provide MD/NP with pertinent information needed for visit and ensure diagnostic testing and medical records are available as needed
Regulate flow of patients as they enter clinic until they are discharged and ensure patient safety
Assist patients with transfers using appropriate ergonomics and safety guidelines
Communicate with patient care team, VNA, outside providers, and supply companies
Order needed supplies for patients, including wound care supplies, pneumatic pumps, and stockings

Responsibilities for Daily Operations of Clinic:

Ensure care team is prepared prior to patient arrival and review chart to have appropriate supplies ready for MD/NP to provide optimal care/assessment
Oversee appropriate infection control practices, including handwashing, donning appropriate PPE, disinfecting of equipment, and sterilizing tools between each patient per CDC recommendations
Disinfect exam rooms between each patient
Ensure exam rooms are stocked with necessary supplies
Return phone calls in a timely manner to patients, providers, and supply companies and complete necessary documentation of phone calls
Ensure safety standards are maintained throughout the clinic
Maintain patient logs, including tissue applications and procedure complications
Ensure future surgical patients are prepared, including diagnostic testing completed and in chart, education complete, and Pre-Op notes complete for providers at least 72 hours from procedure
Ensure timely follow-up on any tests ordered, including labs, wound cultures, biopsy results, and radiology results
Ensure MD/NP is aware of results and that they are available for review
Complete ad-hoc projects as needed

Requirements:

Minimum of ADN/LPN, BSN preferred
Current license, in good standing, as RN/LPN/MA in Massachusetts
Current BLS/CPR certification
Wound care and vascular surgery experience preferred
Ability to work as a member of a team and work collaboratively with other clinicians
Willingness to be available for local and/or regional assignments as needed
Must feel that optimal patient experience and customer service are important
