Hospice Social Worker

As a Hospice Social Worker, you will play a vital role in our interdisciplinary team, providing compassionate care and support to patients and their families.
Key Responsibilities:- Assess patients' and families' social and emotional needs, and develop individualized care plans to address these needs.
- Collaborate with the interdisciplinary team to provide comprehensive care, including pain and symptom management, emotional support, and spiritual care.
- Assist patients and families in navigating the healthcare system, accessing community resources, and making informed decisions about their care.
- Provide education and support to patients and families on advance care planning, end-of-life care, and bereavement.
- Participate in the development and implementation of policies and procedures to ensure high-quality patient care.
- Master's degree in Social Work (MSW) or related field.
- 2+ years of experience in hospice or palliative care, with a focus on social work.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, with a high level of emotional intelligence and empathy.
