Key Responsibilities- Assess patients' and families' social and emotional needs, and develop individualized care plans to address these needs.
- Collaborate with the Interdisciplinary Team to provide comprehensive care, including pain and symptom management, emotional support, and spiritual care.
- Help patients and families navigate the healthcare system, including accessing resources and services.
- Provide guidance and support to patients and families in completing advance directives and making informed medical decisions.
- Participate in the formulation and ongoing evaluation of individualized care plans, and contribute to the development of new care strategies.
- LCSW or LSW licensure
- Master's degree in Social Work
- 2+ years of post-Masters experience in social work, with 1-2 years of experience in hospice care
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
