Join Our Team as a Pediatric Home Care Nurse

We are seeking a compassionate and skilled Pediatric Home Care Nurse to join our team at Phoenix Home Care. As a Pediatric Home Care Nurse, you will have the opportunity to make a meaningful difference in the lives of children and young adults with special needs.

About the Role

As a Pediatric Home Care Nurse, you will provide high-quality, personalized care to children and young adults in the comfort of their own homes. You will work closely with our team to develop and implement individualized care plans that meet the unique needs of each client.

Responsibilities
  • Assist in the development, review, and revision of care plans
  • Provide nursing care in accordance with physician orders and individual care plans
  • Observe, record, and report any changes in client condition to appropriate personnel and/or physicians
  • Provide ongoing instruction and supervision to clients, families, or other healthcare team members
  • Monitor and record patient condition and document provided care services via electronic charting
  • Participate in education programs, quality assessment activities, and staff meetings as required
Requirements
  • Active Licensed Practical Nurse or Registered Nurse License
  • Problem-solving skills and ability to multitask
  • Excellent communication skills
  • Positive, professional attitude
