We are seeking a compassionate and skilled Pediatric Home Care Nurse to join our team at Phoenix Home Care. As a Pediatric Home Care Nurse, you will have the opportunity to make a meaningful difference in the lives of children and young adults with special needs.

About the Role

As a Pediatric Home Care Nurse, you will provide one-on-one care to our clients in the comfort of their own homes. You will work closely with our clients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities
  • Provide skilled nursing care to children and young adults with special needs
  • Develop and implement individualized care plans in collaboration with clients and their families
  • Monitor and record client progress and adjust care plans as needed
  • Provide education and support to clients and their families on disease management and self-care
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements
  • Active LPN or RN license
  • Experience working with children and young adults with special needs
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
