Job Summary

TrueCare is seeking a skilled Behavioral Health Case Manager to provide supportive interventions and linkage to health plan and community resources for patients. The ideal candidate will have a strong background in clinical settings and experience working with behavioral health populations.

Key Responsibilities
  • Provide supportive interventions and psycho-educational interventions to patients
  • Link patients to health plan and community resources
  • Coordinate with other care providers in the community or within TrueCare
  • Assess patient needs and develop individualized plans
  • Monitor patient progress and adjust plans as needed
Requirements
  • High school diploma or equivalent
  • Minimum one year working in a clinical setting
  • Minimum one year working with behavioral health population
  • Bilingual in English and Spanish
  • Computer proficiency with Microsoft Office suite products and electronic health record systems
  • Knowledge of local and national resources related to healthcare
Preferred Qualifications
  • Associates degree in social services, mental health, psychology, human services or related field
  • Experience with brief interventions and case management techniques
  • Experience working with low-income, diverse populations and persons impacted by mental illness
  • CCMC (Certified Case Manager Certification)
