We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Amergis. As a key member of our multi-disciplinary team, you will provide essential social work intervention to patients and clients in need.
Key Responsibilities- Assess patient psychosocial needs and develop plans to meet established goals
- Counsel individuals and families in crisis, addressing issues such as poverty, loss, and illness
- Collaborate with healthcare professionals to plan and evaluate team goals for each patient
- Facilitate referrals to community agencies and provide financial counseling when necessary
- Respond to crisis situations, including domestic violence and child abuse
- Provide witness services and evidence in court cases
- Visit schools to assess academic concerns and work with government agencies to help clients apply for Medicare and Medicaid
- Arrange home visits to evaluate patient health, home environment, and family interaction/support
- Assist patients and families in planning for present and future psychosocial needs
- Report patient progress and changes in patient status to physicians and team members
- Master's degree in Social Work from a Council on Social Work Education-accredited school
- Active license as an LCSW in the state of practice
- Minimum of one year of social work experience
- Compliance with professional standards of practice and Amergis policies
- Participation in Amergis' Competency program
- Current CPR and TB questionnaire, PPD, or chest x-ray (if applicable)
- Health certificate (per contract or state regulation)
- Meet federal, state, and local requirements
- Successful completion of new hire training
- Understand patient confidentiality and HIPAA requirements
- Strong communication skills and proficiency in English
- Computer proficiency
