Hospice Registered Nurse (RN) Job Summary

We are seeking a compassionate and skilled Hospice Registered Nurse (RN) to join our team at Connections Life Care. As a Hospice RN, you will provide high-quality, patient-centered care to individuals and their families in their homes, senior care facilities, and nursing homes.

Key Responsibilities:
  • Provide skilled professional nursing care to patients and their families, as prescribed by the physician and in compliance with Hospice Medicare Conditions of Participation and agency policies.
  • Conduct initial and comprehensive nursing assessments to identify patient needs and develop individualized care plans.
  • Implement and revise care plans to ensure effective, comprehensive, and responsive care.
  • Collaborate with the Interdisciplinary Team (IDT) to ensure all patient and family care needs are met.
  • Supervise Hospice Aides/Homemakers to ensure high-quality care delivery.

Requirements:
  • Graduate of an accredited registered nursing program.
  • Bachelor's degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • Minimum of one year general nursing experience, preferably in hospice, home health, medical, surgical, or critical care.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy and principles of death/dying.
  • Registered Nurse licensed in the Commonwealth of Pennsylvania.
  • Valid driver's license and reliable transportation.
