Job Overview

Key Responsibilities
  • Outstanding Patient Interaction
    • Demonstrate effective communication with diverse individuals, including patients, healthcare professionals, and support staff.

  • Management of Electronic Health Records
    • Efficiently handle inbox messages to ensure timely responses.
  • Telephone Communication
    • Promptly return calls and convey necessary information to patients and relevant parties.
  • Preparation for Patient Visits
    • Assist in preparing patients for medical examinations and procedures.
    • Ensure cleanliness and sanitation of examination rooms post-visit.
  • Patient Care Responsibilities
    • Conduct thorough assessments of patients' health and psychosocial needs, collaborating with them and the healthcare team to develop a treatment plan.
    • Document all interactions in patient records and generate reports on service delivery and outcomes.
    • Collaborate with nursing and clinical operations leadership to gather data for outcome measurement.
    • Maintain accurate patient records and report progress to medical staff.
    • Perform basic laboratory tests and point-of-care testing as required.
    • Assist in medication refills and communicate with pharmacies as needed.
    • Provide care across various demographics, including newborns, geriatrics, and women’s health.
    • Ensure continuity of care by sharing relevant information with various departments.
    • Stay informed about immunization updates and changes.
    • Measure vital signs and conduct necessary screenings.
    • Deliver patient education as required.

Physical Requirements

Employees in this role must be able to sit, stand, and walk for extended periods. They should have good eyesight and manual dexterity. The position may require lifting and transferring patients, as well as performing CPR. Mobility to bend and navigate confined spaces is essential. Exposure to infectious diseases and other occupational hazards may occur.
