Clinical Care Specialist

We are seeking a skilled Clinical Care Specialist to join our team at Maple Crest Health Center. As a dedicated healthcare professional, you will play a critical role in providing high-quality clinical care and services to our residents.
Key Responsibilities:
- Coordinate the delivery of resident care via assessments, planning, intervention, and evaluation
- Develop care plans in collaboration with residents, families, and staff
- Encourage residents to attain and maintain their highest physical, mental, and psychosocial well-being
- Supervise staff in day-to-day activities

Requirements:
