Care Coordinator

Job Summary:
We are seeking a highly skilled and compassionate Care Coordinator to join our team at Amcord Care Inc. As a Care Coordinator, you will play a vital role in identifying, coordinating, and facilitating supportive services and community resources for our elderly and disabled participants.
Key Responsibilities:
- Attend and complete orientation training and ongoing annual training
- Manage an active caseload, monitoring and evaluating options and services to meet an individual's health needs
- Complete person-centered assessments and assist members in obtaining HCBS services that support independent living
- Inform and assist members with gaining access to needed LTSS and Medical Assistance services
- Complete monthly monitoring telephone calls and develop and modify service plans as needed
- Maintain ongoing communication with providers and participants
- Ensure participants exercising participant-directed budget authority do not exceed approved service hours
- Report all member incidents and allegations of neglect/abuse within 24 hours
- Follow Amcord Care Inc. policies and procedures for document maintenance, confidentiality, and employee rights
Requirements:
- Bachelor's degree in social work, psychology, or related field
- OR at least 3+ years of experience in a social service or healthcare-related setting
- Reliable transportation
- Bilingual preferred but not required
