Job Summary:
We are seeking a highly skilled Diabetes Care Specialist to join our team. As a Certified Diabetic Educator, you will play a vital role in providing comprehensive care to diabetic patients through individual consults and classes.
Job Description:
The successful candidate will be responsible for:
Conducting individual assessments and working with family/support persons of diabetic patients. Developing and implementing individual and group educational sessions to meet the assessed needs of patients, in accordance with American Diabetes Association guidelines. Providing clinical expertise and understanding diabetes treatment plans, goals, and objectives of the Diabetic Collaborative Project. Establishing and maintaining professional relationships with key providers and integrating information regarding patient needs between interdisciplinary team members. Facilitating communication between the interdisciplinary diabetes care team members and patients. Actively involved in health prevention, primarily weight management, exercise, and proper nutrition.Required Skills and Qualifications:
- Well-organized with ability to motivate, educate, and work with individuals possessing a wide range of skills and backgrounds.
- Ability to work independently with very little supervision.
- Good communication, listening, motivation, education, problem-solving, and counseling skills.
VNA Health Care offers a competitive salary range of $65,000 - $80,000 per year and a comprehensive benefits package, including health insurance, paid time off, and professional development opportunities.
