Job Title: RN Admissions Clinical Assessment Advisor

Join CenterWell Home Health, a leading provider of home care services, as a RN Admissions Clinical Assessment Advisor. In this role, you will play a critical part in ensuring the highest quality of care for our patients.

Key Responsibilities:
  • Conduct comprehensive clinical assessments of patients to identify their needs and develop personalized care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Develop and implement care plans that address the physical, emotional, and social needs of patients.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience with a minimum of one year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
