The Enhanced Care Management (ECM) Case Manager RN, under the guidance of the Case Management Manager, is tasked with addressing both clinical and non-clinical requirements of members facing the most intricate medical and social challenges. This role emphasizes systematic service coordination and comprehensive care management.
ECM is designed to be interdisciplinary, highly interactive, and centered around the individual, primarily delivered through in-person engagements with members in their preferred environments.
Key ResponsibilitiesThe case manager will work with individuals experiencing chronic health issues, homelessness, or those at risk, as well as those with frequent hospital admissions, substance use disorders, behavioral health needs, or transitioning from incarceration.
Utilizing exceptional communication skills, the case manager will:
- Provide services and coordination to ensure seamless continuity of care across health and social service programs.
- Collaborate with the member's interdisciplinary team (ICT) to support their needs.
- Engage members and their support systems to identify priorities that align with their desired outcomes.
Licensure: Must possess a current RN license in California.
Experience: A minimum of one year in a Registered Nurse capacity within an acute care hospital, mental health environment, outpatient clinic, or healthcare agency.
Knowledge: Familiarity with current nursing principles, medical terminology, patient care protocols, and community resources is essential.
Skills: The ability to perform comprehensive nursing duties, develop and implement care plans, maintain accurate records, and foster effective relationships with healthcare professionals and patients is crucial.
Interpersonal Skills: Must demonstrate strong analytical thinking, attention to detail, and excellent communication abilities.
Team Collaboration: A willingness to work autonomously and as part of a team to achieve shared goals is required.
Additional RequirementsPossession of a valid driver's license and proof of state-required auto liability insurance is necessary.
